Admission Criteria for Seniors' Choice, Adult Day Programs

The following criteria are required for admission to the Seniors' Choice Adult Day Programs. If you have questions about any of the items listed, please do not heistate to call us.

• Able to perform most activities of daily living with assistance
• No current history of wandering or able to remain within facility with minimal redirection
• Able to transfer with 1-person assist, accommodation for a 2-person assist will be determined on an individual basis
• Fifty years and older
• Able to communicate needs (with or without speech)
• Absence of combative or disruptive behaviors
• Free from any infectious or communicable diseases
• Responsive to bowel and bladder program
• Able to bring prescribed medications in daily, to be dispensed by Med Certified staff
